Participant hereby explicitly and unambiguously consents to the collection, use and transfer, in electronic or other form, of Participant's personal data as described in this Option Award Agreement and any other Option grant materials ("Data") by and among, as applicable, the Employer or other Service Recipient, Employer, the Company and any Parent or Subsidiary for the exclusive purpose of implementing, administering and managing Participant's participation in the Plan. Participant understands ...that the Company and the Employer may hold certain personal information about Participant, including, but not limited to, Participant's name, name; home address and address; telephone number, numbers; date of birth, birth; age; social insurance number, social security number, taxpayer identification number or and/or other identification number, salary, nationality, number; tax related information; salary; salary history; nationality; job title, title; any Shares shares of stock or directorships held in the Company, Company; details of all Options Restricted Stock Units or any other entitlement to Shares shares of stock awarded, canceled, exercised, vested, unvested or outstanding in Participant's favor ("Data"), favor; and benefit enrollment forms; for the exclusive purpose of implementing, administering and managing the Plan. Participant understands that Data will be transferred to a such stock plan service provider as may be selected by the Company in the future, from time to time, which is assisting the Company with the implementation, administration, administration and management of the Plan. If Participant does not consent, or if Participant later seeks to revoke his or her consent, his or her engagement as a Service Provider employment status or service and career with the Employer will not be adversely affected; affected thereby; the only adverse consequence of refusing or withdrawing Participant's consent is that the Company would not be able to grant Participant Options Restricted Stock Units or other equity awards or administer or maintain such awards.
