Form of Letter to Exercise Nonqualified Stock Option
This document is a template letter for employees or option holders to exercise their nonqualified stock options under a Stock Option Agreement. The individual specifies the number of shares to acquire, the exercise price, and the method of payment (cash, stock, broker sale, or share withholding). The letter also requests issuance of a stock certificate in the individual's name. The parties involved are the option holder and the issuing company. The letter must be completed, signed, and submitted to the company to formally exercise the stock option rights.
