Extension Letter to Executive Transition Agreement between RTI Surgical, Inc. and Robert P. Jordheim
RTI Surgical, Inc. and Robert P. Jordheim, its Executive Vice President and CFO, have agreed to extend the term of Mr. Jordheim's Executive Transition Agreement, originally dated August 29, 2012. The agreement, which was set to expire, will now remain in effect through December 31, 2018, with all other terms and conditions unchanged. Both parties have confirmed their acceptance by signing this extension letter.

Your Executive Transition Agreement with RTI Surgical, Inc., dated August 29, 2012, is about to expire.
