Subscription Agreement between Larkspur Health Acquisition Corp. and Larkspur Health LLC (May 7, 2021)
Larkspur Health LLC has agreed to purchase 1,494,998 shares of Class B common stock from Larkspur Health Acquisition Corp. for $17,333. The agreement requires Larkspur Health LLC to pay this amount and accept the shares under the company's Certificate of Incorporation. The transaction is formalized by both parties' signatures and will be recorded in the company's stockholder ledger.

In consideration for the issue of the Shares, the undersigned hereby agrees and undertakes to pay $17,333 to the Company.
