Amendment to 2006 MRP Vision Plan Coverage Terms

Summary

This document outlines amendments to the 2006 MRP Vision Plan, changing the frequency and monetary limits for lenses, frames, and contact lenses. The new terms reduce the number of pairs and increase or decrease the dollar limits for certain items, clarify that members must choose between lenses or contacts, and expand coverage to include special lens coatings. Medical coverage is now limited to in-network providers only. These changes affect all plan participants and are effective as specified in the amendment.
