Celladon Corporation Executive Retention Program Summary (April 2015, as Amended)
Celladon Corporation established a retention program for certain current and former executive officers. Under this program, eligible executives will receive a lump sum payment equal to 50% of their base salary if they remain employed with Celladon until December 31, 2015, or if they are terminated without cause before that date. Pursuant to Celladon Corporations retention program, the executives officers and former executive officers listed below will be eligible to receive, or have received, a lump sum retention payment equal to 50% of his or her base salary upon such individual remaining employed by Celladon until December 31, 2015, or such individuals earlier termination without cause prior to such date:
