Abbott Change in Control (CIC) Agreement Extension Notification to Executive
Abbott has notified the executive that their Change in Control (CIC) agreement, which provides financial, health, and welfare benefits if there is a change in control of the company, has been extended. The agreement, originally set to expire on December 31, 2016, will now remain in effect until December 31, 2018. No action is required from the executive to maintain participation in the agreement.

Abbott’s Board of Directors recently extended your Change in Control (CIC) agreement. The CIC agreement provides you with financial, health and welfare benefits in the event of a Change in Control. No action is required on your part to continue participation in the CIC agreement.
You are hereby notified that your current Change in Control Agreement, which was set to expire on December 31, 2016, has been extended to December 31, 2018.
